The
Hospice Experience
Hospice care is person centered and compassionate, taking into consideration
the unique way that each individual approaches death. We are aware that
the dying process is as unique as the individual experiencing it. Hospice
care is provided by compassionate, end-of-life trained staff. We provide
care and support as well as pain and symptom management along with assistance
in decision making. Our passion is to meet the desires of both patient and
family with spiritual, emotional and social support. The end-of-life nursing
team at Valley View Nursing Center considers it an honor and a privilege
to be a part of, and share in, the very personal and emotional experience
of the dying process - both with the patient and the family.
Palliative Care
When you or your loved one faces
a serious disease, we offer the services of Palliative Care. This is an
interdisciplinary team approach that concentrates on reducing the severity
of disease symptoms to provide relief from suffering.
Advanced Care Planning and the Physician
Orders For Life-Sustaining Treatment (POLST) Form
Your desires and beliefs are important
to us. Our certified Advanced Care Planning facilitators can assist you
and your family to express your decisions and wishes regarding end-of-life
care. We work along with your physician to assure that your specific needs
are met.
